Robotic Radical and Partial Nephrectomy

Robotic nephrectomy is a minimally invasive surgical procedure used to remove part or all of a kidney when a tumor or other abnormality is present. Depending on the tumor’s location, size, and complexity, Dr. Friedman may recommend either a partial nephrectomy (removal of the tumor and surrounding tissue) or a radical nephrectomy (removal of the entire kidney).

Both procedures are performed using the da Vinci robotic system, which allows for enhanced precision, smaller incisions, and faster recovery compared to traditional open surgery.
 

Understanding the Surgical Decision: Partial vs. Radical Nephrectomy

Choosing between a partial or radical nephrectomy is a complex, individualized decision. Dr. Friedman follows a structured approach to guide treatment planning, based on the following priorities:

  2. Oncologic Cure – Complete removal or destruction of the tumor

While imaging and planning provide important information before surgery, intraoperative findings may lead to adjustments—such as converting a partial to a radical nephrectomy if needed to ensure safety or complete tumor removal.

What to Expect During Surgery

  • Performed under general anesthesia using small incisions

  • The robotic system provides enhanced dexterity and visualization for precise dissection

  • For partial nephrectomy, the tumor is carefully removed while preserving surrounding kidney tissue

  • For radical nephrectomy, the entire kidney (and sometimes the adrenal gland or nearby lymph nodes) is removed


Hospital Stay and Catheter Use

  • All patients stay at least one night in the hospital

  • A urinary catheter is placed during surgery and removed the following day

  • Pain is managed with a combination of oral and IV medications

  • Walking is encouraged on the evening of surgery to reduce complications and support recovery


Drain Use in Partial Nephrectomy

In partial nephrectomy patients, a surgical drain may be placed at the time of surgery to monitor for bleeding or urine leakage:

  • The drain exits through a small incision and is secured to the skin

  • It is typically removed before hospital discharge or at your first post-operative visit

  • If present, we provide clear instructions on how to care for the drain at home


Recovery Timeline

  • Hospital stay: Typically 1 day, depending on recovery and complexity

  • 1–2 weeks: Return to light activities; lifting restrictions apply

  • 4–6 weeks: Most patients return to full activity

  • Follow-up imaging and labs: Monitor kidney function and ensure proper healing

Risks and Side Effects

While robotic nephrectomy is generally well tolerated, potential risks include:

  • Bleeding or need for transfusion

  • Injury to nearby organs or vessels

  • Infection or urine leakage (more common with partial nephrectomy)

  • Decreased kidney function, especially if the remaining kidney has preexisting issues

  • Need for conversion from partial to radical based on findings during surgery
