Prostate Cancer Screening

Screening aims to detect prostate cancer at an early, more treatable stage. The two main tools for screening are:

  • Prostate-Specific Antigen (PSA) blood test: A simple blood test that measures PSA levels, which may be elevated in prostate cancer as well as other prostate conditions such as enlargement or infection. PSA levels can be variable, so confirmatory testing is often necessary.

  • Digital Rectal Exam (DRE): A physical exam where the prostate is felt through the rectal wall to check for abnormalities.

These are nuanced screening tools that help identify men who may need further evaluation. The diagnosis of prostate cancer is not made based on PSA or DRE alone. Not every man requires screening, and the decision is individualized based on age, family history, race, and overall health. Biopsy & Evaluation for Prostate Cancer

If screening results suggest a higher risk of prostate cancer, further evaluation is recommended. This may include:

  • Repeat PSA testing to confirm elevated levels

  • Prostate MRI to identify suspicious areas

  • Prostate biopsy to obtain tissue samples for microscopic examination

Prostate biopsies are typically performed using ultrasound visualization with MRI guidance to improve accuracy. The biopsy provides the most definitive information about the presence and aggressiveness of prostate cancer.

Prostate Cancer Management 

Treatment for prostate cancer depends on factors such as the cancer’s stage, aggressiveness, and the patient’s overall health and goals. Options may include:

  • Active surveillance: Careful monitoring with regular exams, PSA tests, and repeat biopsies for slow-growing, low-risk cancers.

  • Robotic Radical Prostatectomy: Removal of the prostate  for localized cancers.

  • Radiation therapy: External beam radiation or brachytherapy (radioactive seed implantation). 

    • Some patients may be candidates for a rectal spacing procedure prior to radiation to help reduce potential side effects​

  • Focal therapy: Newer approaches, such as cryotherapy or high-intensity focused ultrasound (HIFU), aim to treat only the affected portion of the prostate. While these options are not offered at our practice, they are available at select centers and may be appropriate in certain cases.

  • Hormone therapy, chemotherapy, or newer targeted therapies: Typically reserved for advanced or higher-risk cancers.

For some men, especially those with slow-growing cancers, active surveillance may be the best approach to avoid unnecessary treatment while still maintaining close monitoring.
