Penile Prosthesis Implant

A penile prosthesis implant is a safe, effective surgical treatment for men with erectile dysfunction (ED) that does not respond to oral medications, injections, or vacuum devices. This procedure involves placing a device inside the penis to allow for controlled, natural-feeling erections.

What Is a Penile Implant?

The three-piece inflatable penile prosthesis consists of:

  • Two inflatable cylinders placed inside the penis

  • A pump placed in the scrotum

  • A fluid reservoir placed in the lower abdomen

When the pump is squeezed, fluid moves from the reservoir into the cylinders, creating a firm, natural erection. Pressing a release valve allows the fluid to return to the reservoir, deflating the device.

This device is completely internal, undetectable to others, and provides on-demand erectile function with high satisfaction rates.


Who Is a Candidate for a Penile Implant?

Penile implants are typically recommended for men with:

  • Moderate to severe erectile dysfunction

  • Failure or intolerance of oral medications (PDE5 inhibitors), injections, or vacuum devices

Preoperative Optimization

To minimize the risk of complications, especially infection, patients are carefully evaluated and optimized before surgery. This includes:

  • Lowering A1C levels in diabetic patients to under 8.0%

  • Weight loss if BMI is elevated

  • Addressing smoking cessation, urinary infections, or other medical issues

  • Reviewing all medications and ensuring proper skin hygiene

The Procedure

  • Performed under general anesthesia

  • Outpatient or overnight stay depending on health and complexity

  • Small incisions are made in the scrotum and lower abdomen

  • The device is placed entirely within the body

  • Patients are given antibiotics to reduce infection risk


Recovery Timeline

  • Days 1–3: Swelling and discomfort managed with pain medications and scrotal support

  • 1 week: Return to light activity; avoid sexual activity and heavy lifting

  • 3–4 weeks: Most patients feel significantly improved; swelling subsides

  • 4–6 weeks: Follow-up with Dr. Friedman for device activation and teaching

  • 6 weeks and beyond: Patients may begin using the device for intercourse

Dr. Friedman provides step-by-step guidance on how to use and maintain your device once healing is complete.


Risks and Potential Complications

While satisfaction rates are high, penile implant surgery carries certain risks:

  • Infection – most serious complication; may require device removal

  • Bleeding or hematoma

  • Device malfunction (rare; may require revision)

  • Erosion or mechanical failure over time
