Hydrocelectomy or Spermatocelectomy

A hydrocelectomy is a surgical procedure to remove a hydrocele, which is a fluid-filled sac surrounding the testicle. Hydroceles are typically benign, but when they become large or uncomfortable, they can affect daily activities and quality of life.

What Is a Hydrocele?

A hydrocele occurs when fluid collects around the testicle, usually within the scrotum. It may develop:

  • Gradually over time (often due to aging or gravity)

  • After injury or infection

  • As a congenital condition (more common in infants, but sometimes persists or recurs in adults)

Most hydroceles are painless but may cause:

  • Scrotal swelling

  • A feeling of heaviness or dragging

  • Discomfort with activity, clothing, or intimacy

When Is Surgery Needed?

Not all hydroceles require surgery. However, hydrocelectomy may be recommended if:

  • The hydrocele is large or growing

  • It causes discomfort, pressure, or difficulty with movement

  • There is uncertainty in diagnosis and tissue needs to be evaluated

  • You simply prefer to have it treated

During your consultation, we’ll perform a physical exam and scrotal ultrasound if needed to confirm the diagnosis and help you make an informed decision.

The Hydrocelectomy Procedure

  • Typically performed as an outpatient surgery

  • Done under general or regional anesthesia

  • A small incision is made in the scrotum or groin to drain the fluid and remove or close off the sac

  • The area is closed with dissolvable sutures

You’ll be able to go home the same day with clear aftercare instructions.

Recovery After Hydrocelectomy

Most men recover quickly, with manageable discomfort. Here's what to expect:

  • Swelling and bruising are common and may last 1–2 weeks

  • The area of the previous hydrocele may swell and have a similar appearance as prior to surgery. This is expected. The body should reabsorb this fluid over a period of weeks. It does not mean the procedure has failed. 

  • Supportive underwear and ice packs help minimize swelling

  • Avoid heavy lifting or strenuous activity for 2–3 weeks

  • Full healing typically occurs within 4–6 weeks

Long-Term Outcomes

Hydrocelectomy is generally curative, with low risk of recurrence. Once healed, most men report:

  • Resolution of swelling

  • Improved comfort with daily activities and intimacy
